Our Mission
“To provide East Harlem/El Barrio with high quality, comprehensive, accessible and culturally sensitive preventive and primary care services, and to coordinate the other levels of health care that are necessary.”

History
Settlement Health has provided high-quality, comprehensive primary health care services to East Harlem since 1977. Originally established by Union Settlement, Settlement Health is now governed by a Board of Directors made up primarily of East Harlem residents.

In May 1999, Settlement Health completed construction of a three-floor Health Center, which was designed with an eye toward attractive, bright, inviting waiting rooms, and comfortable setting for its patients. Settlement Health has been guided by a strong belief that our patients - the residents of East Harlem - deserve the same comforts and quality of care that exists in the city's finest health centers.  We are pround to take teh lead in ending the tradition of noisy crowded "clinics" with long waits as the only health care option for our community. 

Background
The health center operates at 212 East 106th Street (between 2nd and 3rd Avenues) - and the Health Education Department is at 2082 1st Avenue @ 108th Street. The center employs experienced physicians, nurse practitioners and certified nurse midwives, offers services on two evenings and Saturdays, and provides 24-hour telephone coverage. New York State has designated Settlement Health a Preferred Primary Care Provider, and the federal government has designated the center a Federally Qualified Health Center.

Settlement Health has been serving the community of East Harlem since 1977. We are a non profit community health center dedicated to providing high quality primary medical care to all who seek and need it. We also offer specialty care referrals through our affiliation with Mount Sinai Hospital.

The health center currently employs over 80 individuals, and provides over 45,000 health care visits annually to 12,000 individuals. The health center has a budget of approximately $7.3 million. Federal, state, city and private funding account for 40% of operating funds, with patient and user fees accounting for the rest.

Services
Settlement Health provides a full range of primary care services: prenatal, pediatrics, adolescent, adult and geriatric care which is delivered in three Unit Areas; Pediatric (1st Floor), Women’s Health (1st Floor) and Adult (2nd Floor) Units. Specialty services are routinely coordinated with other physicians and local hospitals (Mount Sinai, North General and Metropolitan Hospitals). The health center also offers individual consultation in the areas of nutrition, GYN, and diabetes. In addition to these primary health care services, Settlement Health offers numerous supportive and educational services, including HIV counseling and testing services, comprehensive primary care services and case management services for those who are HIV+ or have AIDS, HIV/AIDS support groups and activities, parenting groups, childbirth classes, and a range of support and educational groups for the community.

Our high quality services are available to citizens of all countries who reside in the Greater New York area, regardless of age, race, gender, language, origin, religion, or disabilities.

The Health Education Department Settlement Health is a leading provider of HIV/AIDS outreach and education services in East Harlem. The program provides dozens of educational presentations to individuals and groups in the community each year, and is a leading advocate for HIV services in East Harlem. Some of the activities include: street level outreach to the community at large; general community outreach; mass outreach to individuals at high-risk to link them with C&T and primary care; targeted presentations and workshops to women and men at risk through their substance use or unprotected sexual activities; & targeted presentations to adolescents in schools and in after school programs; development of low-literacy prevention education materials; and coordination of community forums on issues relating to HIV prevention and care. All primary and secondary HIV prevention education, risk assessment & skill building occurs with a view to initiating and assisting participants with developing harm/risk reduction practices and to achieving some level of behavior change. Evidence of the health center’s effectiveness in this regard is evident in the numbers of people reached through street outreach and community education during 2001: Over 30,000 individuals were reached through our HIV outreach efforts - including street level encounters, targeted outreach to difficult populations and structured events.
